| 在供给侧的视角下，根据传统慢性病管理模式，创新建构“互联网+”慢性病管理体系，集可穿戴式测量仪、慢病患者、移动医疗、慢性病管理系统、专业技术人员、远程医疗以及政府为一体，通过体系建构帮助慢性病患者及高危人群实现预防、治疗和预后，做好患者的自我管理，患者的统一管理，有利于提高慢性病知晓率和控制率，辅助慢性病管理人员管理慢性病患者，促进慢性病防控工作有效开展。 |
