军队乙型肝炎疫苗预防接种成本-效益分析

摘要:
目的 通过对目前军队采用的乙型肝炎疫苗全员接种方案与筛检后分类接种方案的经济效益比较分析,为制定更加精准的军队乙型肝炎疫苗免疫策略提供科学依据。方法 采用TreeAge Pro V2011建立 两种方案的决策树模型,通过查阅文献获得乙肝相关疾病发病概率、疾病年经济负担等参数信息,代入成本-效益计算公式分别计算两种接种方案的年成本、效益、净效益(N)值和效益-成本(BCR)比,并比较两者差异。结果 若军内每年需要接种乙肝人数为20万人,全员接种方案和筛检后分类接种方案的成本分别为312万和235.73万元,两者效益为547.85万和641.04万元,两者净效益为228.98万和398.51万元,效益-成本比为1.76和2.72。结论 从 卫生经济学角度分析,说明采用这两种方法在经济学上都是可行的,并且都能产生正效益,但采用筛检后分类接种是更加经济可行的方法。

Abstract:
Objective: To use the method of benefit-cost analysis to compare the existing immunization scheme with the pre screening one. So we can lay a foundation for the formulation of the more economical and effective immunization strategy. Methods: We established 2 decion tree models of those 2 immunization strategy by TreeAge Pro V2011.To find out the parameter information such as the morbidity of hepatitis B related disease, disease burden ,and so on, through the literature. The data were generated into the cost-benefit formula in order to calculating the annual cost, benefit, net benefit and benefit cost ratio of 2 vaccination scheme separately and comparing the gap between them. Results: If the PLA recruits 200 thousands each year, the costs of existing vaccination and vaccination after screening were 3.23 million yuan and 2.36 million yuan with benefit 5.48 million yuan and 6.41 million yuan. It is as the result that the net benefit of existing vaccination and vaccination after screening were 2.29 million yuan and 3.99 million yuan with benefit-cost ratio 1.76 and 2.72 respectively. Conclusion: The results indicated that screening followed by vaccination was obvious superiority and more recommended.
