| 设想组建公立医疗机构联合体，通过优质资源下沉，帮助基层提升服务能力，从而留住患者，推动分级诊疗，这不过是卫生行政部门良好的主观愿望，客观上作用有限。第一，组织结构决定当前多数医联体不可能以分级诊疗为目的；第二，在市场经济条件下三级大医院分级诊疗动力不足；第三，在无竞争压力的情况下基层医疗机构工作缺乏积极性；第四，大医院专家难以真正解决基层医疗困局。 |

| Construction of public medical association through sinking high-quality resources to the grass-roots level, so as to enhance their service capabilities ,to retain patients, and to promote the classification treatment, is good wishes by the administrative department of public health Subjectively. It is limited objectively. First, current organizational structure decides the majority of the current medical association can not be classified for the purpose of hierarchical diagnosis. Second, three level hospitals lack the motivation to put forward hierarchical diagnose. Third, the basic medical institutions lack enthusiasm in the absence of competitive pressure.Fourth, it’s difficult for experts of big hospitals to solve the dilemma of primary medical institutions. |
