| 目的：用因子分析探讨黑龙江省医保制度设计存在的问题及影响因素，提出相应的政策建议。 方法：通过文献分析建立调查问卷并展开调查，使用SPSS软件进行因子分析。 结果：因子分析筛选出的4个公因子揭示了困扰医保制度设计的关键问题之所在，首先是公平性因子，主要由医保制度筹资与支付标准以及覆盖程度等设计不合理因素所驱动；第二公因子为管理失效因子，主要受医保制度现有监督及管理体系内部设计不合理影响；第三公因子为参保方式因子，主要是医保制度设计中个人参保方式对医保制度实施产生影响；第四公因子为制度衔接因子，主要是医疗保障体系与医疗体系衔接设计不合理。 结论：医保改革应重点围绕医保制度设计中的突出问题，完善相关制度设计；加快对医保现行管理体系、监管与评价的改革设计；此外，还应高度关注医疗保障与医疗两大体系的有机衔接、完善个人参保方式等多个领域的系统设计改革来实现全民医保的政策目标。 |

| Abstract: |
| Objective To explore the problems and influencing factors of the design of medical insurance system in Heilongjiang Province by factor analysis. Methods Through literature survey and investigation to establish questionnaire and conducting investigations, with the survey data used SPSS package for factor analysis. Results The four common factors derived from factor analysis reveal the key issues that hinder medical insurance system design.First,the fairness factor is driven by the imbalance among fund raising,financing and payment standard and degree of insurance coverage.Second,the failure management factor is affected by present ineffective supervision of medical insurance system and inappropriate interior design of management system.Third,insurance mode factor mainly refers to the effect that individual insurance made has exerted on the medical insurance system implementation .Fourth,the system connecting factor involves the ill-connection between medical insurance system and health system. Conclusion Medical insurance reform should center on the striking issues in medical insurance design,improve related system design and quicken the reform design in present management system,supervision and evaluation.Besides special attention should be paid to the connecting system between medical insurance system and health system.Individual insurance mode and other related areas should be perfected. With all these,the aim of all citizens medical insurance will be realized. |
